Summary

This position provides guests that are exiting the program and into housing, the support needed for sustainability in maintaining permanent housing. To determine needs for the after-care service, the specialist works with each person to develop a personalized plan. Most services are field-based and require travel to and from the guest’s permanent home. The specialist also makes necessary referrals for community services that a former guest might need.  The right person is someone who should possess excellent people skills and be a proactive listener, be compassionate and have empathy.

Job Duties

  • Provides case management (referrals, service coordination, goal identification)
  • Conducts home visits as determined by the needs of guests
  • Assists guests with identifying community services for continued success in maintaining stable housing
  • Develops an after care plan with guests prior to their exit from shelter
  • Advocates for guests as they build their worn support network in the community.

Skills/Qualifications

  • Bachelor’s Degree in Human Services preferred
  • Minimum 3 years’ experience in a similar role
  • Proven experience in the development and implementation of programs
  • Proficient in Microsoft Outlook, Word, Excel and PowerPoint   

Working Conditions

  • Regular standing, walking, climbing, crouching, bending, pushing, or pulling
  • Understand, speak, read, and write fluent English
  • Required to talk and  hear
  • Able to lift up to 50 lbs.
  • Able to use fine motor hand functions
  • Frequently exposed to varying temperatures from below 32 degrees to above 80 degrees
